Coming Home Connection means care, resources and companionship for older adults in Santa Fe.

Coming Home Connection is here for older adults in need of a little extra support as they age. Each of our five programs – Taking Care of Neighbors no-cost caregiving (“TCN”), Resource Navigation, The Loan Closet, Volunteer Companions, and Traditional Caregiving – is a part of our holistic approach to meeting the needs of older adults in Santa Fe. As we approach the end of 2025, our services are reaching hundreds of neighbors:

  • TCN provided no-cost caregiving to 45 older adults living alone
  • Resource navigation has been provided to 120 clients
  • Over 1,500 neighbors have borrowed medical equipment, more than 3,000 pieces of equipment loaned
  • Volunteer companions are working with 14 older adults
  • More than 100 people have received in-home care for short-term, on-going or respite needs

Our vision is for older adults to be empowered to live their best lives, age in their place of choice, and be happy and healthy. We are being asked to help more people each year as our population ages. Please join us and help make Santa Fe a wonderful place for older adults to call home!
